Our office accepts most major health insurance plans. Please remember that many health plans require a referral from a primary care physician. We recommend that you check with your individual insurance plan before scheduling an appointment to verify your benefit coverage.

As a professional courtesy, we will request office referrals from your PCP; however, if your insurance plan changes and/or you change your primary care physician, it is important that you contact us with this information as soon as possible.

Billing Your Procedure

There are four (4) separate charges involved in a procedure, and you may receive up to four (4) different bills:  one from the facility (surgical center or hospital), one from Dr. Dumois for the physician’s fee, one from the anesthesiologist, and one from the lab if a biopsy is performed.

If you schedule a procedure, please contact your insurance company to find out if you have a co-pay, deductible and/or coinsurance. Your colonoscopy and/or endoscopy is considered and billed as an outpatient surgical procedure. It is done at an outpatient surgical center or at a hospital, not in the doctor’s office.

Unless prior arrangements are made, all co-pays, co-insurances and/or deductible balances for the surgery center are due in full on the day of your procedure. A billing fee will be charged to all accounts not paid in full at that time.  Again, please contact your insurance company to determine the amount you will be responsible for. You can find the customer service telephone number on the back of your insurance card. If you have been scheduled for a preventative “screening” colonoscopy, this means that you have no signs or symptoms and that you do not have a history of polyps.  Your insurance plan has a set benefit for preventative screenings, and the procedure is usually covered if your insurance is in-network.  However, if Dr. Dumois finds a polyp or abnormality during the procedure, your benefits may change and your insurance might pay differently.  Upper endoscopies (EGD) do not have a “screening” benefit with insurance companies.

Again, please contact your insurance company to determine if your procedure will be covered.  The representative in member services will ask you for the procedure code and diagnosis codes for your procedure.  The procedure code to verify insurance benefits for a colonoscopy is 45380.  Please access our patient portal to view your consultation visit summary to obtain the diagnoses codes that will be used to file the claim on the day of your procedure.
